Under regular supervision and following established policies and procedures, locates, retrieves, scans and delivers medical records for patient care, release of medical information, research, audits, or administrative purposes. Updates chart locations via the computerized tracking system. Requests, receives, and scans medical records from off-site storage facility and DFCI clinics. Scans requested medical record documentation into electronic document management systems.

Responsibilities

  • Scan requested medical records for scheduled and STAT appointments according to established time frames.
  • Access patient index, chart tracking systems, and manual systems to prep and scan records. Verify patient records numbers as necessary.
  • Provide quality assurance for prepped and scanned documents.
  • Contribute to teamwork and perform other duties as required by the position.
  • Follow established department and Institute policies, procedures, and work rules.
  • Follow established schedules of chemo and loose material pickup at various clinics.
  • Processes charts ordered from offsite facility according to appointment date.

Qualifications

  • 1-2 years of data entry experience
  • 1-2 years experience in a clerical setting preferably in healthcare or a high production environment.
  • High school education.
  • Computer, data entry and proofreading skills.
  • Ability to index and file electronically.
  • Ability to gather and interpret data in situations in which the information or problems are straight forward. 
  • Ability to read, understand and respond to simple written or verbal instructions.
  • Incumbents must respond appropriately to instructions, questions or request for information.
  • Must be able to distinguish colors on color-coded medical record folders.
  • Ability to review the medical record and identify glaring discrepancies with the content and other documentation.
