  • Overview

    The Pharmacists are responsible for the optimization of drug therapy by identifying, resolving, and preventing drug-related problems and adverse drug events.  The clinical pharmacy specialist in this position will be working with the ambulatory stem cell transplant service.

    • Patient medication teaching and medication reconciliation pre and post transplant
    • Staff and clinical team education and providing drug information

    Clinical Pharmacy Services

    • Detection of drug-drug interactions
    • Evaluation and monitoring drug therapy
    • Participating in clinic conferences, and patient care rounds
    • Managing the transitions of care


    • Reviewing standards and ensuring compliance with Joint Commission, FACT and other regulatory standards that apply to stem cell transplant

    Committee Responsibilities

    • Participation in the Pharmacy & Therapeutics Committee
    • Development, implementation and maintenance of stem cell related medication and treatment guidelines
    • Chemotherapy and stem cell treatment medication order set review and approval
    • Participation in various stem cell transplant committees



    • Bachelor of Science in Pharmacy or Doctor of Pharmacy degree from an accredited college of Pharmacy and must be licensed to practice pharmacy in the Commonwealth of Massachusetts. 
    • 5 to 7 years of hospital pharmacy or retail pharmacy experience is required as well as a minimum of 4 years of oncology experience in a specialized area (stem cell transplant preferred) or completions of an ASHP accredited PGY2 residency in Oncology.  Must be Board Certified in Oncology Pharmacy (BCOP) or advanced degree related to the specialty practice area is preferred.
    • Must maintain the continuing education credits required for stem cell transplant accreditation.
